Pizza Order Form – Teams should submit pizza order by Friday, Feb.3rd,2017. Payment for pizza should be made in cash the morning of the competition. Pies will be delivered to your homerooms starting at 11:00AM. If you order a large number of pies you will most likely receive your pizza in two deliveries. This will allow us to get food to everyone in a timely manner.
Street Map – Buses should NOT park in the school parking lot. It is WAY to small! After drop off, drivers can park buses in the county parking lot, which is adjacent to the school.
DAHS Map – Here is a view of the building but we will provide maps the morning of competition as well.
INVY Check-In Forms – The following forms must be turned in upon arrival.
Code Of Conduct Form One form for EACH TEAM
FINAL REGISTRATION FORM – One form for EACH TEAM
Vandalism Policy One form per SCHOOL
Medical Form must be shown for each student but will be held by the coach
Medical Form – One for EVERY STUDENT IN ATTENDANCE
Publicity Release Form– One for EVERY STUDENT IN ATTENDANCE
